(Aetna (NOT AETNA CVS), Anthem BCBS & FEP, Medicare, , Humana PPO, Empire BCBS, Cigna (not Cigna Connect), United Health Care PPO Plan, Blue Cross PPO (NOT SELECT), Tricare (requires prior authorization)
WE DO NOT ACCEPT MEDICAID OR HMO PLANS.
If you have any questions about our participation with your insurance plan, please call our office.
If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor before your visit.
Our online scheduler allows patients to self-schedule up to 90 days into the future. If you are an established patient, you should see a “welcome back!” notice appear after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, please call the office after scheduling your appointment to verify your information, before filling out the registration form which will be e-mailed to you.
For Surgical or Cosmetic appointments, you will need to contact our office to schedule.
For Facials with Laurie, our Aesthetician, please use the online scheduler to schedule your appointment
Registration
After scheduling an appointment, you will receive a registration link from eDerm.
Login with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t login, please call the office.
New Patients:
New Patient Forms are found on our website home page under patient forms. We ask that you print and complete these forms and bring with you at the time of your visit. Please be sure to bring your photo ID and insurance card(s) with you to your appointment.
If you are a new patient, we ask that you arrive 15 minutes ahead of your appointment.
